Office Policies

What You Should Know Before your visit

We are committed to providing you with the highest quality care and service. A clear understanding of our financial policies is essential to ensure a smooth experience with your treatment. We are happy to work within the guidelines of your insurance plan; however, it is your responsibility to be familiar with those guidelines.
Our office hours are Monday – Thursday: 8:00 AM – 4:30 PM & Friday: 8:00 AM – 4:00 PM

Office Policies & Patient Information

Appointments: We make every effort to see patients at their scheduled time and kindly ask that you do the same. If you anticipate being late, notify us immediately. If you arrive more than 10 minutes late, you may be rescheduled for the next available appointment. To help prevent delays, we ask all patients to arrive 15 minutes before their appointment to allow time for check-in and any necessary updates to your medical records.

Cancellation & No-Show Policy If you need to cancel or reschedule an appointment, please provide at least 24 hours' notice. Failure to do so will result in a fee: $50.00 for office appointments & $100.00 for diagnostic testing. Missed appointments take away opportunities for other patients who need care. If you miss two appointments within a 12-month period without notice, you may be discharged from our practice. If you fail to show for your first appointment and do not call to cancel, we may not reschedule it.

Doctor-Patient Communication: We strive to provide clear explanations of your condition, medications, and treatment options. If you have any questions, do not hesitate to ask. Our team is here to support you in a welcoming and efficient environment.

Prescriptions & Refills: For routine prescription refills, please have your pharmacy fax a request during regular office hours. We aim to process requests within 24 hours. Do not wait until you are out of medication to request a refill. Some prescriptions require a physician visit before renewal. We do not process prescription renewals over the phone after office hours.

Returning Messages Our trained staff can address many of your concerns. If your call requires a physician’s response, we will return it as soon as possible, typically within 48 hours. Non-emergency calls should be made during business hours.

Emergencies (Evenings, Weekends & Holidays): Dr. Fix provides care during regular office hours but does not offer hospital care or emergency visits outside these times. If you experience a medical emergency, call 911 or visit the nearest emergency room. We will gladly coordinate with treating physicians and share necessary medical records.

Financial Agreement & Insurance Policies: All Professional Fees Are Due at the Time of Service. Patients must notify the front desk of any insurance or address changes. Patients are financially responsible for any charges if insurance changes are not communicated before the appointment. Our office files insurance claims on your behalf, but services are billed to the patient, not the insurance company. You are responsible for co-pays, deductibles, co-insurance, non-covered services, and any procedures deemed “not medically necessary” by your insurance. If a claim remains unpaid for more than 90 days, the balance is due in full. Patients must follow up with their insurance carrier. Patients will be notified of financial balances via statement, patient portal, text, or phone call. Payments should be made promptly to avoid collections. A $50 fee applies to any returned checks.

Auto Insurance & Personal Injury Protection (PIP): If PIP benefits are available, we will submit claims to your auto insurance. We do not file PIP claims with third parties. Patients are responsible for 20% co-insurance at the time of service unless they provide secondary health insurance. Updating Personal Information. To ensure smooth communication and billing, please keep us informed of any changes to your address, phone number, employment, or insurance. Patients must update their information annually or whenever a change occurs.

Forms & Medical Records Requests: Disability, financial, and other medical forms require 5-7 business days for processing. Some forms may have a processing fee. Medical records are confidential. We will not share information with family, friends, or employers without written consent. Record requests require a signed release form and are processed within 15 business days. Fees apply: $1.00 for the first page, $0.50 for each additional page, plus postage (per Florida law). Payment is due before records are released.
